Get an eye exam.

Complete the certificate and attach the required number of box tops, AND attach receipt(s) of the eye exam and product purchase.
Mail completed certificate, box tops and required reciept(s) to the address (and before date) specified on certificate.
Keep a copy of your paperwork for your records.
