When your order is shipped to you, your rebate certificate will be included in your order
Complete the certificate and attach the required number of box tops, attach receipt(s) of the eye exam and product purchase.
Mail completed certificate, box tops and required receipt(s) to the address (and before date) specified on certificate.
Keep a copy of your paperwork for your records.
Please allow 6 to 8 weeks for delivery of your rebate check.
