rebate coupons redeemable from
the manufacturer.
Claiming Your Rebate
- Get an eye exam.
- Purchase the required number of boxes of Freshlook Brand Contact Lenses
from
CoastalContacts.com.
- When your order is shipped to you, your rebate certificate will be
included in your order
- Complete the certificate and attach the required number of box tops,
attach receipt(s) of the eye exam and product purchase.
- Mail completed certificate, box tops and required receipt(s) to the
address (and before date) specified on certificate.
- Keep a copy of your paperwork for your records.
- Please allow 6 to 8 weeks for delivery of your rebate check.
Rebate Details
- Offer is limited to one (1) rebate per person.
- Original mail-in certificate, original sales receipt and copy of valid
eye exam receipt are required. Rebates will be sent only to street or rural
route addresses, P.O. Boxes will not be honored.
